Afterbirth+ is the second expansion pack for The Binding of Isaac: Rebirth. It features new content and long awaited built-in mod tools and documentation.
Unused Graphics#
Backdrops#
Outer Corners#
Some outer corner sprites were graphically updated in Afterbirth+, but still remain unused.

Blue Womb Entrance |
Bearded Ultra Greed#
Image file: \gfx\bosses\afterbirthplus\deliriumforms\afterbirth\boss_ultragreed_beard.png
Image file: \gfx\bosses\afterbirthplus\deliriumforms\afterbirth\boss_ultragreed_body_beard.png
In early screenshots of Afterbirth, Ultra Greed was chubbier and had a beard resembling the one Tyrone Rodriguez had at the time. None of these sprites were present in Afterbirth. Their placement in the deliriumforms folder is very odd, considering Delirium is never fought in Greed Mode.
Early Ultra Greed#
Image file: \gfx\bosses\afterbirthplus\deliriumforms\afterbirth\boss_ultragreed_body.png
Also in the deliriumforms folder are sprites of Ultra Greed with something closer to his final design, though still a bit more plump and with a few sprites left incomplete.
Ultra Greedier#
Image file: \gfx\bosses\afterbirthplus\boss_ultragreedier.png
Ultra Greedier's sprite sheet, which is mostly redrawn from Ultra Greed's, includes the intro sprite that shows him hanging from the ceiling. There's never a situation where this is needed.
Big Horn's Big Bomb#
Image file: \gfx\bosses\afterbirthplus\boss_bighorn.png
Big Horn has an unused big bomb. Nothing else is known about it. Bombs of similar size, known as Giga Bombs, would later be utilized in Repentance.
Sideways Lil' Delirium Headless Baby#
Image file: \gfx\familiars\delirium\familiar\familiar_shooters_269_headlessbaby
Lil' Delirium's Headless Baby form has sprites for it facing sideways, like the regular version. They go unused for the same reasons.
Inactive Forgotten#
Image file: \gfx\characters\costumes\character+017_theforgotten
The Forgotten's sprite sheet has an unused sprite that shows it lying on its stomach, intended to be used when playing as The Soul. According to the developers, this went unused due to animation required for switching between the sprites being too long and complex for what was supposed to be an instant transition.
Forgotten's Grave#
Image file: \gfx\effects\dirtpatch
Sprites for the dirt patch that unlocks the Forgotten when dug up with We Need To Go Deeper!, colored brown to match the color scheme of most other floors. The dirt patch that appears can only appear in Dark Room, which uses a black version of this sprite to match the ground on that floor.
Afterbirth+ Room Editor#
The room editor included in Afterbirth+ has many sprites differing from the actual game, presumably leftovers from Rebirth and Afterbirth's development.

Notes |
|
|---|---|---|---|
Mask of Infamy Heart |
Less realistic, has more prominent veins. |
||
Mask + Heart Heart |
Less realistic. |
||
Camillo Jr. |
Colored with rosy pink instead of black. It's possible Camillo Jr. wasn't intended to be exclusive to the Negative route originally. |
||
Gurgling (Enemy) |
Guts are half red and half beige instead of fully red. |
||
Begotten |
Seems to use similar assets to the final version, but they're very sloppily assembled in the early sprite. They use the default pink body. |
||
Mega Fatty |
Rounder, more purple, lacks the blood around his eyes and mouth, has a different face, and seems to be larger. This early design formed the basis for his Last Will portrait. |
||
The Cage |
Bigger than the final sprite, and at a different angle. |
||
Uriel |
Uriel and Gabriel's halos were higher up on their heads and had either different or nonexistent glowing effects. Their Fallen variants may or may not have had white wings. |
||
Gabriel |
|||
Fallen Angel |
|||
Closed Stone Maw |
Uses the same sprite as its Broken variant. |
||
Bloodshot Eye |
Resembles clouded-over versions of the default Eye instead of being free-standing eyeballs. This probably means they weren't originally going to appear in Sheol. |
||
The Husk |
Less shiny and the flies inside its mouth are less defined. |
||
Key Ring |
The keys are at uneven angles. |
||
Mega Satan |
Has a red tint. |
||
Greed Mode Button |
When active, the Greed Mode button was yellow and had no spikes, which indicates that pressing it didn't hurt the player at some point. |
||
Hush |
Hush originally had X's for eyes, like ???. |
||
Meatball |
The Meatball's inner skull was beige-colored instead of gray, which is inconsistent with the Host enemy. |
||
Portal |
Portals were red, yellow, and white instead of purple. This may have been changed to match the Void item. |
||
Door |
Doors were less detailed and had a different color palette. |
Dollar Icon#
Image file: \gfx\ui\main menu\difficultywidget
Stored with the graphics for the difficulty selection is a dollar sign icon, likely meant for Greedier Mode. In the final game, a simple exclamation mark is used for it.
Hidden Signature#
Image file: \gfx\items\pick ups\pickup_016_multibombs.png
To the right side and bottom of most the effects is text labeling what each effect is for, and two of the ones at the bottom goes unused with text saying "SAD UNUSED", and "BIG SAD UNUSED" next to it. Lower right corner has a signature that says "LACAP" and "FALTA DAÑO" (Spanish for "missing damage").
Unused Audio#
Inhale#
The sound of something inhaling.
