The developers ran into trouble when designing Yennefer, as she was not described in great detail in the books. They created 4 models before settling on the final version, which they were very happy with.
One of the reasons that Yen and Ciri weren't introduced in the earlier games was that the developers weren't confident about their ability to tackle such multifaceted characters. (For example, Yen being a mix of really mean and really sweet.) As a result, they felt they had to hone their skills before they could do the characters justice.
