As expected, Repentance was playable at the PAX West Convention, with some gameplay being uploaded to GameSpot's YouTube channel, from here, several differences can be pointed out.
- Collecting pennies still produces the sound used in Afterbirth+
- Shop doors still have the Afterbirth+ appearance, simply being an edited version of the regular closed doors with a lock.
- The Crispy enemies still use their Afterbirth+ appearance.
- Baby Plum's boss portrait uses the one from Antibirth, instead of the updated design in the final game.
- Isaac's character portrait still uses the one from Afterbirth+
- The Baby Plum fight itself is slightly different, and seems to be slightly faster than the one in the final game.
- Treasure Rooms in the alternate floors only have one item available, as opposed to two, with one of them being hidden.
- An unused arena for the Pride miniboss is fought, being in one of the thinner rooms found in Afterbirth+. The same goes for the Wrath boss fought later in the video.
- The red ghosts made by the Vade Retro look slightly different from the ones found in the final game.
- The Blighted Ovum boss still uses its Afterbirth+ sprites.
- The ground decorations in the Downpour floor use the same assets as the ones in the Basement.
- A portal enemy appears in one of the rooms of Downpour, which never happens in the final version.
- The Goat Hoof trinket still uses its art from Afterbirth+
- The bosses in the alternate floors use the normal fight music in the normal floors, instead of Juggernaut.
Additionally, in a tweet depicting the new Dross floor, Blue Baby can be seen with its Afterbirth+ design.
